A doctor’s referral is required for all exam services we perform, so inform your doctor that you choose HDI for your medical imaging studies.
Ways to Make Your Appointment With HDI:

Visit our office at 976 Mountain City Highway in Elko, NV, and speak with our Front Desk/Patient Scheduler. Our office is located in the A+ Total Care building; visit our contact us page for directions.
Have your physician fax their order to 775.621.5801. Upon receiving your fax, we will follow up with you and your doctor to coordinate your appointment.
Make an appointment online using our appointment request form and we will followup with you to discuss further details.
